What Is Forward Head Posture?

Forward Head Posture is a postural pattern in which the head rests in front of the shoulders when viewed from the side. The position changes how the neck, upper back, and surrounding muscles share movement and load. It may appear during desk work, device use, driving, or other sustained positions.

How Forward Head Posture Works

A person examines a man’s neck posture beside a monitor showing “FORWARD HEAD POSTURE”

Forward Head Posture describes a change in the head and neck position during standing, sitting, or movement. A clinician may view the person from the side and compare the head position with the shoulders, upper back, and pelvis. The assessment also considers whether the position changes during movement.

The position can alter coordination between the cervical spine and upper back. Neck muscles may work differently when the head stays ahead of the shoulder line, especially during desk tasks or device use. A posture correction plan may address movement habits, joint motion, muscle control, and workstation demands together. This approach connects the observed position with the activities that reproduce it.

From the Advanced Injury Care Clinic team

When we assess Forward Head Posture, we observe alignment at rest and during simple movements. We also consider the neck, upper back, shoulder position, and daily activities that may influence the pattern. Our team uses those findings to guide Posture Correction work without treating posture as an isolated measurement.

Forward Head Posture in Practice: A Real-World Example

A person who spends much of the workday looking down at a laptop may show the head positioned ahead of the shoulders during a standing check. The clinician may then examine how the neck and upper back move during ordinary tasks.
